Accession Application

Accession Applicant – Applicant must be SELECTED OR EAD on or after 1 Oct 09 and before 1 Aug 10 to be considered eligible to APPLY.  AFPC/DPAM (D,N,W) must receive original signed ADHPLRP Service Agreement within 10 days of applicant being SELECTED OR EAD – deadline for receipt is 1 Aug 10.

  2. Accession ADHPLRP Application
  3. Lender Release Form - for each lending institution.
  4. Loan Summary Sheet - for each lender (if loans were consolidated applicant must submit copies of original loan documentation and consolidation document with application package)
  5. Accession ADHPLRP Service Agreement
  6. Squadron Commander's Certification Letter - This is the final enrollment document. To be completed after arrival at permanent duty station and submitted within 30 days. For nurses that need to complete Nurse Transition Program, the Squadron Commander's letter must be completed and submitted within 30 days of successful NTP completion.
  7. National Practitioner Data Bank - Attach to Commander's Certification Letter, if applicable. Recommend applicant go to credentials department at the facility where you work for copy of NPDB or see NPDB Web Site for self query instruction.
  8. License Validation Form - To be completed by credentials department at permanent duty station and attach to Commander's Certification Letter.
  9. Participant receives e-mail when payment has been authorized for disbursement to DFAS
  10. Participant contacts AFIT/ADHPLRP if payment not received within 60 days.

Summary

Applicant must submit the below documents to the field recruiter to apply for ADHPLRP

  1. Original Accession ADHPLRP Application
  2. Original Lender Release Form(s)
  3. Original Loan Summary sheet(s)
  4. Original Accession ADHPLRP Service Agreement (Service Agreement must be signed and received before 01 Aug 2009) by AFPC/DPAM (D,N,W)

Applicant must submit the below enrollment documents to AFIT/ADHPLRP within 30 days of arrival
at their permanent duty assignment after completion of COT and NTP (if NTP required).

  1. Original Commander’s Certification Letter (letter must be signed by the Squadron Commander)
  2. Original License Validation Form (form must be completed and signed by the credentials or education & training department).
  3. National Practitioner Data Bank printout (if applicable)

AF Representative

  1. Field Recruiter reads AFIT/ADHPLRP web page
  2. Field Recruiter confirms number of Corps accession allotments available, applicable AFSCs
  3. Field Recruiter forwards application documents to HQ AFRS selection board
  4. HQ AFRS confirms applicant SELECTED OR EAD. HQ AFRS notifies AFIT/ADHPLRP within 10 Days applicant being SELECTED OR EAD
  5. HQ AFRS notifies Applicant/Field Recruiter of ADHPLRP conditional selections/rejections
  6. HQ AFRS sends original ADHPLRP Service Agreement to respective corps AFPC and original PIF folder to AFIT/ADHPLRP
  7. AFPC confirms receipt of original Service Agreement from HQ AFRS within 10 days of applicant being SELECTED OR EAD before 1 Aug 09

AFIT/ADHPLRP

  1. AFIT/ADHPLRP verifies folder items - contact conditionally selected participants
  2. ADHPLRP prospectively validates loans for amount and eligibility, determines prospective award amount.
  3. AFIT/ADHPLRP obtains actual payoff values, actual validated amount (award)
  4. AFIT/ADHPLRP creates voucher for payment and sends to DFAS, notifies participant
  5. AFIT/ADHPLRP investigates payments overdue 60 days
