Transcript Request
The Registrar’s Office maintains official AFIT transcripts. These permanent records are confidential, and are not released without written permission of the student.
Official transcripts bear the signature of the Registrar and the seal of the institution. There is no charge to obtain official transcripts.
To obtain your transcript, or have it sent to an institution, submit the following information:
- Name (to include maiden name or other previous names)
- Social Security number
- Date of Birth
- Dates of Attendance
- Current address and phone number
- Address(es) where you would like your transcript(s) sent
Don’t forget to include a signature authorizing release of the transcript.
Requests can be faxed to (937) 255-2791 (DSN 785-2791), or mailed to the address below.
