If you have completed a course from LS and wish to obtain a transcript (at no cost) you may mail or FAX (can not accept email due to signature requirement) a letter requesting a transcript with the following information:

addresses where the transcripts are to be mailed
class(es) taken
dates (copies of your certificates are very helpful)
daytime phone number
